پیش‌بینی ریسک قطع عضو بیماران دیابتی با استفاده از الگوریتم‌های بهبودیافته یادگیری ماشین

چکیده
قطع عضو اندام تحتانی یکی از جدی‌ترین عوارض دیابت است که بار اقتصادی، اجتماعی و روانی سنگینی بر بیماران و سیستم‌های بهداشتی تحمیل می‌کند. هدف این مطالعه، توسعه و ارزیابی الگوریتم‌های بهبودیافته یادگیری ماشین برای پیش‌بینی ریسک قطع عضو در بیماران دیابتی با تمرکز بر تفسیرپذیری و قابلیت کاربرد بالینی است. به منظور انجام این پژوهش، الگوریتم‌های مختلف یادگیری ماشین شامل XGBoost، LightGBM، Random Forest،SVM ،KNN ، شبکه های عصبی عمیق و مدلهای ترکیبی، بر روی داده های بیش از ۳۳۰ هزار بیمار دیابتی توسعه یافتند. برای بهبود عملکرد مدل ها، تکنیک‌های مهندسی ویژگی، متعادل‌سازی داده، بهینه‌سازی بیزی ابرپارامتر و روش‌های Ensemble به‌کار گرفته شدند. تفسیرپذیری مدل ها با استفاده از SHAP و LIME و تحلیل اهمیت ویژگی ارزیابی گردید. نتایج حاصل از مطالعه حاکی از آن است، مدل XGBoost با AUC برابر 93/0 (اعتبارسنجی داخلی) و 84/0 (اعتبارسنجی خارجی) بهترین عملکرد را در پیش‌بینی قطع عضو نشان داد. مدل LightGBM با AUC برابر 9۰/0، 85/0 و 76/0، به ترتیب برای پیش‌بینی عدم قطع عضو، قطع عضو مینور و قطع عضو ماژور دست یافت. تحلیل SHAP نشان داد که شمارش گلبول‌های سفید، شاخص CCI، شمارش گلبول‌های قرمز، تغییرات اندازه گلبولهای قرمز خون، گانگرن، استئومیلیت، بیماری عروق محیطی، عفونت سیستمیک و کاهش وزن مهم‌ترین پیش‌بینی‌کننده‌ها هستند. مدل ترکیبی با استفاده از Weighted Boxes Fusion بهmAP برابر با 864/0 در تشخیص زخم دست یافت که 4/12 درصد بهتر از benchmark مطالعات قبلی بود. درنهایت می توان نتیجه گرفت، الگوریتم‌های بهبودیافته یادگیری ماشین با دقت بالایی می‌توانند ریسک قطع عضو در بیماران دیابتی را پیش‌بینی کنند. استفاده از روش‌های تفسیرپذیری، شفافیت لازم برای پذیرش بالینی را فراهم می‌کند. مدل‌های توسعه‌یافته می‌توانند به عنوان ابزارهای غربالگری اولیه برای شناسایی بیماران پرخطر و هدایت تصمیمات بالینی مورد استفاده قرار گیرند.

Predict Amputation Risk in Diabetic Patients with Using Improved Machine Learning Algorithms

Lower extremity amputation is one of the most serious complications of diabetes, imposing a heavy economic, social, and psychological burden on patients and healthcare systems. The aim of this study was to develop and evaluate improved machine learning algorithms for predicting the risk of amputation in diabetic patients, with a focus on interpretability and clinical applicability. To conduct this research, various machine learning algorithms, including XGBoost, LightGBM, Random Forest, SVM, KNN, deep neural networks, and ensemble models, were developed on data from more than 330,000 diabetic patients. To improve model performance, feature engineering techniques, data balancing, Bayesian hyperparameter optimization, and ensemble methods were applied. Model interpretability was assessed using SHAP, LIME, and feature importance analysis. The results of the study indicate that the XGBoost model demonstrated the best performance in predicting amputation, with an AUC of 0.93 (internal validation) and 0.84 (external validation). The LightGBM model achieved AUCs of 0.90, 0.85, and 0.76 for predicting no amputation, minor amputation, and major amputation, respectively. SHAP analysis showed that white blood cell count (WBC), CCI score, red blood cell count (RBC), red cell distribution width (RDW), gangrene, osteomyelitis, peripheral artery disease, systemic infection, and weight loss were the most important predictors. The hybrid model using Weighted Boxes Fusion achieved an mAP of 0.864 in ulcer detection, which was 12.4% better than the benchmark of previous studies. Finally, it can be concluded that improved machine learning algorithms can predict the risk of amputation in diabetic patients with high accuracy. The use of interpretability methods provides the necessary transparency for clinical acceptance. The developed models can be used as primary screening tools for identifying high-risk patients and guiding clinical decisions.
